You pose them a question or otherwise elicit a response, and they type it in.
It's private, so only the sequence monitors can read it.

*Question* and *Answer*
You pose a bunch of questions to Learners, who can respond in free text.
It's not really assessable, as we don't know what they're going to write, but you can choose to make certain questions compulsory, and give generic feedback for each question.
What's *really cool* is that after the answers are submitted, you can choose to have LAMS display *All* of the answers provided by learners, and you can then have them rate and comment on each answer. Of course, this can be anonymous or not.

The Survey tool polls users with either single, multiple, or free text responses.
It all gets collated and displayed to learners and the sequence monitors.

The mind map is a bit of a new tool, and it basically creates a nice mind-map widget that learners can add balloons to around a central topic.
It can be set up as an individual or group activity — although the groups need to be kept small, as it can quickly get out of control.

Allows learners to submit 'records' that you define in authoring, so, sort of like creating a database.
The records can consist of any sort of field: dates, drop down lists, checkboxes.
It may be useful for collecting data about the delivery standard of services.
